Why Anaesthesia-Free Dental Cleaning Isn’t Safe for Cats

Quick answer: Anaesthesia-free dental cleaning only scrapes the visible surfaces of the teeth. It cannot clean below the gumline (where the damaging disease actually lives), cannot allow proper X-rays or examination, and tends to stress the cat. Worst of all, it makes teeth look clean while disease progresses underneath, giving false reassurance. A scale and polish under general anaesthesia remains the veterinary standard.

The idea of cleaning your cat’s teeth without anaesthesia sounds appealing: no anaesthetic, lower cost, less worry. It is understandable that owners are drawn to it. But as vets, we see the harm it can do, and the major veterinary bodies advise against it. The problem is not the intention; it is that anaesthesia-free cleaning simply cannot do the job that matters. Here is why.

What anaesthesia-free cleaning actually does

In an anaesthesia-free procedure, someone restrains the awake cat and scrapes tartar off the parts of the teeth they can reach, essentially the outer, visible surfaces. The teeth come away looking whiter, which is exactly what makes the service so persuasive.

But cleaning teeth is not really about appearance. The whole point of a professional clean is to remove disease-causing bacteria from where they do damage, and that is below the gumline, in the pockets around each tooth and on surfaces you cannot see. An awake cat will not, and cannot, tolerate the instruments going there.

Why “below the gumline” is the whole point

Periodontal disease (the most common dental problem in cats) does its damage beneath the gum. Plaque and tartar creep into the space between tooth and gum, where bacteria attack the bone and ligaments that hold the tooth in place. You can read more in our guide to periodontal disease in cats.

A clean that only addresses the visible crown leaves all of that untouched. The teeth look better, but the disease marches on, which is why an anaesthesia-free clean can be worse than doing nothing, because it hides the warning signs that would otherwise prompt proper treatment.

The hidden risks

Beyond simply being ineffective, anaesthesia-free cleaning carries real downsides:

  • Stress and fear. Restraining an awake cat while scraping at sensitive, possibly painful teeth is frightening and uncomfortable: the opposite of the gentle, low-stress care cats need.
  • Risk of injury. A struggling cat can be hurt by sharp instruments, and sudden movements risk soft-tissue damage.

  • False reassurance. This is the biggest danger. Owners believe the problem is handled, so a genuinely diseased mouth goes untreated for months or years.

Why anaesthesia is the safer choice

It feels counter-intuitive, but general anaesthesia is what makes a proper, safe clean possible. Under anaesthesia the cat feels nothing, stays completely still, and the airway is protected so water and debris cannot be inhaled. The vet can probe every tooth, clean thoroughly below the gumline, and treat any disease found, all in one calm, controlled visit (with referral for dental X-rays where a case needs them).
